Which Method Should You Choose?

The Old-Fashioned Way:

Here’s the traditional way to do it: You first fill out the application form (PDF file) posted on the website of the Wyoming Department of Health.

To request a birth certificate in Wyoming, you must be 18 years old (to request your own certificate), a parent of the person listed on the certificate, a lawyer representing one of the aforementioned parties, or a legal guardian.

The fee for a Wyoming birth certificate is $13. A money order or personalized check should be payable to Vital Records Services. The check must be in the name of the person ordering the record—third-party checks will not be accepted.

Be sure to include a valid, current copy of your driver’s license, passport, or state-issued ID card. These must have a visible signature and expiration date. Alternatively, you can have your signature on the application notarized.

You must also include a self-addressed stamped envelope with your request.
