Two Ways to Get a Birth Certificate in Wisconsin

The Old-Fashioned Way:


If you opt to obtain a Wisconsin birth certificate the old-fashioned way, you can start by downloading the application and filling it out. You must be able to prove you have a relationship to the individual or have a valid legal interest in requesting a certificate; follow the directions provided on the form. The state issues certified and uncertified birth certificate copies.

You are also required to provide photocopies of valid identification. There are two options:


  • Option 1: Send one (1) of these:
    • Wisconsin driver's license or ID card
    • A driver's license or ID card issued by another state

  • Option 2: Send two (2) of these:
    • Government-issued (U.S.) photo ID
    • Checkbook or bank statement
    • Health insurane card
    • Passport
    • Current, sigbned lease (must be dated)
    • Paycheck or earnings stateemnt
    • Traffic ticket
    • Utility bill
A birth certificate in Wisconsin costs $20, and additional copies are only $3 each if these are requested with the original order. A personal check or money order can be made payable to Wis. Vital Records. Please include a business-size self-addressed stamped envelope (SASE) with the application.

  • Fax instructions (less preferred): Sign AND date a plain piece of paper, and place your government issued ID under or next to the signature. Enlarge and lighten the ID by 300%, and fax to us. This is less effective than the above.
