MONTANA BIRTH CERTIFICATE
Which Method Should You Choose?
Two Ways to Get a Birth Certificate in Montana
The Old-Fashioned Way:
You have several options if you’re curious about how to get a birth certificate in Montana. The traditional method involves sending an application via postal mail. You can start by downloading the application (PDF) from the state website.
To request your own birth certificate, you must be 14 years of age or older. To request the certificate of another party, you must be their spouse, child, parent, grandparent, caretaker relative, guardian, authorized representative, or someone able to provide documentation showing a compelling need for the record.
You must provide a copy (front and back) of a valid photo ID. Acceptable forms of identification are a driver's license, passport, state ID card, military ID card, or tribal ID. If none of these is available, consult the list of alternative options mentioned on the application.
A certified copy of a birth record costs $12, and additional copies are $5 each. Montana also provides "informational" certificates ($10) that are available to anyone provided that the birth took place at least 30 years before the date of the application. Fill out a check or money order in the correct amount (U.S. funds only) payable to Montana Vital Records.
Mail everything to:
Office of Vital Records
Department of Public Health and Human Services
111 N Sanders Rm 6
PO BOX 4210
Helena, MT 59604
It takes 2-3 weeks to process your application once it has been received.
