Which Method Should You Choose?

The Old-Fashioned Way:

First, you need to complete the Iowa Department of Public Health's application form to request a certified document.

The application must be signed in front of a notary public. You are also expected to provide a copy of a current government-issued photo ID in your name. In addition, you must state the purpose of your request.

The fee for a state-certified Iowa birth certificate is $20. You can make a check or money order out to the Iowa Department of Public Health.

It can take up to six weeks to process your request for an Iowa birth certificate.
