Which Method Should You Choose?
Two Ways to Get a Birth Certificate in Illinois
The Old-Fashioned Way:
To order Illinois birth certificates, you must be the person named on the document, a parent named on the document, or a legal representative of the person. The first step is to fill out the Application for Illinois Birth Record (PDF).
You must also provide current identification documents. You have multiple options:
Option #1: A copy of a valid (unexpired) government-issued photo identification card, such as a driver's license or a DMV ID. You must include both sides if there is information on each.
OR
Option #2: If you have an ID that expired less than 6 months ago, you must include ONE additional document that shows your name and current address. The additional document must be dated within the last 6 months.
OR
Option #3: If you have an ID that expired more than 6 months ago, or if you have no ID at all, you must include TWO documents that are dated within the last 6 months. You must submit one item from each of these two groups:
Group #1 (select one of the following): Medical card, voter's registration card, auto insurance card, paycheck stub (with full personal information), bank statement, financial statement, or credit card statement.
AND
Group #2: A piece of recent mail that shows your full name and current address. Acceptable examples include a phone bill or a utility bill.
PLEASE NOTE: Social Security cards are NOT acceptable identification.
You need to send a check or money order payable to IDPH.
$15 is the price for a certified long-form birth certificate, which can be used for all applicable legal purposes. A short-form birth certificate costs $10. Each additional certified copy costs $2.
