Two Ways to Get a Birth Certificate in Florida
The Old-Fashioned Way:
How do you get a Florida birth certificate? One way to do it is by contacting the Florida Department of Health, Bureau of Vital Statistics. Please note that to obtain a birth certificate in Florida, you must be the person named on the document, be the parent or guardian of that person, or have a valid court order. You also must include with your application a copy of a current photo ID, such as a driver's license, state or military identification card, or passport.
You can begin the process by downloading the official application. The form asks for detailed information about the requester as well as the person listed on the birth certificate. You must have a copy of an acceptable, current photo ID: a driver's license, a state or military identification card, or a passport.
Mail the above and a $9 fee (check or money order payable to Vital Statistics) to the following address:

The $9 fee is for a "computer certification" that will be accepted by any state and federal agency. Alternatively, the requester can order a photocopy certification for $14.
