You are required to send a copy of a valid government-issued photo ID or passport; if neither is available, consult the list of alternative documents printed on the application form.
To request a birth certificate, you must fall into one of these categories:

• A child or grandchild of the person (must be at least 18)
• The spouse, parent/guardian, or grandparent of the person
• A state or federal employee (must have signed release from subject of certificate)
• An attorney representing the person, or representing their parent/guardian, spouse, or child
• The chief elected official of the locality where birth took place
• A local Director of Health of the locality where birth took place
• A member of a genealogical society recognized by the State of Connecticut
Certified Connecticut birth certificates cost $30 each. You must pay with a postal money order; personal checks are not acceptable. Money orders should be payable to Treasurer, State of CT.
