• A legal representative of any person mentioned above (must provide proof)
If your name is not on the birth certificate, then you must send along proof of relationship, such as a marriage certificate or other legal document.
You also need to supply a copy of appropriate identification. A valid photo driver's license or U.S. passport will do the trick (must be unexpired); if these are not available, see the complete list of acceptable documents found on the application. You can obtain Colorado birth records dating back to 1910.
A certified copy of a birth certificate in Colorado costs $20. Additional copies (if ordered with the initial copy) are $13 each. Fill out a check or money order payable to Vital Records. Cash will not be accepted.

- Fax instructions (less preferred): Sign AND date a plain piece of paper, and place your government-issued ID under or next to the signature. Enlarge and lighten the ID by 300%, and fax it to us. This is less effective than the above.
- Mail instructions (last resort): Mail a copy of your government-issued ID with a signature AND date to us. (Please note that this copy will not be returned to you.)
