- Department of Corrections photo ID card, with probation/discharge papers dated less than a year ago
- Employment identification with photo, accompanied by a current pay stub or W-2 form
- If requester is currently incarcerated, a Department of Corrections photo ID card, with verification of incarceration from the facility on official letterhead
- Current student photo ID, with a current report card or current transcript (requester must be at least 15 years of age)
If none of the above are available, the requester can send copies of at least THREE of the following documents (one of which must be dated within the last year):
- Expired state or federal photo ID, marriage certificate, divorce certificate, child’s birth record, W-2, paycheck stub, voter registration, bank statement, car registration, health insurance or Medicaid card, utility bill, baptismal certificate, dentist/doctor/hospital bill, state benefit card, letter from a government agency such as the IRS or Social Security office, Social Security benefit statement, tax record, school record, incarceration record, or land/rental agreement
