Which Method Should You Choose?
Two Ways to Get a Birth Certificate in New Jersey
The Old-Fashioned Way:
There are two types of New Jersey birth certificates. A certified copy will bear a raised seal and be printed on official State of New Jersey safety paper. The other type is a certification; this is printed on plain paper and cannot be used for legal purposes (e.g., establishing identity).
Anyone requesting a New Jersey birth certificate of a non-family member will receive a certification, not a certified copy. To request a certified copy, you must provide proof that you are (1) The subject of the birth certificate, (2) Their parent, legal guardian, or legal representative, (3) Their spouse/civil union partner, domestic partner; child, grandchild, or sibling, if of legal age, (4) A representative of a state or federal agency on official business, or (5) A possessor of a court order.
First, fill out the application provided by the New Jersey Department of Health.
You will be expected to send copies of appropriate identification. There are two options:
(1) A copy of a current photo driver’s license
OR
(2) Two of the following (one must display current address):
A certified copy or certification of a birth certificate in New Jersey costs $25; additional copies are $2 each, provided that they are requested in the initial order. Please make checks or money orders payable to Treasurer, State of New Jersey.

What You'll Need?
New Jersey requires the applicant to submit identification, a signature, and a date in order to receive a birth certificate. Identification must be government issued, such as a driver license, state ID, or passport. Instructions are below.

Take a photo of the ID/signature with a smartphone or camera, and email that to us or text it to us. Alternatively, you may scan and email the ID/signature to us if you'd like. PLEASE DO NOT FORGET THE DATE!

- Fax instructions (less preferred): Sign AND date a plain piece of paper, and place your government issued ID under or next to the signature. Enlarge and lighten the ID by 300%, and fax to us. This is less effective than the above.
- Mail instructions (last resort): Mail a copy of your government issued ID with a signature AND date to us (Please note that this copy will not be returned to you.)
